Use the registration form on this page to submit your registration.
We created a Health Practitioner account during the practitioner registration process that allows you to access members’ health information and laboratory test results.

Once logged in, go to the ‘Practitioner Health Profiles’ section.

Once in your profile, you can search for a specific member using their unique PIP Code in the Search Console Section.
Accessing Member Health Profiles
To facilitate seamless access, each member's health profile is now linked to a unique QR code. We have introduced a convenient feature that allows you to access a member’s health profile by scanning a QR code. Each member’s device is equipped with a QR code-generating API that links directly to their Personal Identification Profile (PIP) code. Here’s how you can use this feature:
Steps to Access and Upload.
Using QR Codes for Easy Access.
1. Scan the QR Code: Use your mobile device or QR code scanner to scan the QR code displayed on the member's device.
2. Enter the PIP Code: Once scanned, you will be prompted to enter the PIP code provided by the member into the Search Console within your Health Practitioner Profile section in your account.
3. Access Health Profile: After entering the PIP code, you will gain access to the member's comprehensive health profile, including their laboratory test results.
4. Upload Samples: Follow the on-screen instructions to securely upload any collection samples to the member's profile directly from your device.
This system will significantly streamline accessing and updating member health information, thereby improving the efficiency and quality of care you provide.
